Review refill requests on your own time.
The Refill Clinic is a technology platform. Independent licensed practices review patients' cash-pay requests to continue a current medicine or repeat care they have had before, and are paid weekly for the decisions they make.
- MD, DO, NP or PA
- No appointments
- Paid weekly to your practice

The admin is ours. The judgement is yours.
An asynchronous queue
No appointments and no scheduling. Paid requests wait in your state's queue with the patient's answers, uploads and a drafted summary. Preview a case, then take only the ones you choose.
You decide every item
Sign, decline, ask the patient a question, order labs, send an item to a concierge visit or mark an emergency, one item at a time. You can always decline; an AI score never blocks or decides anything.
Your practice, your letterhead
Your practice stays independent and is the provider of record for the care it delivers. Prescriptions carry your practice's letterhead, never ours, and go to the pharmacy the patient chose.
The admin is handled
Intake, patient email and payment collection, refunds, receipts, e-prescribing and fax, and a weekly settlement to your practice. You keep clinical judgement; the platform never directs it.
Fixed clocks, so nobody waits.
Clocks are fixed so patients are never left waiting and nobody is rushed into a decision.
- 24hours
To decide a case
Taking a case gives you 24 hours to decide its items. Deciding items one at a time does not change that deadline.
- 48hours
For the patient to reply
Asking the patient for information pauses the case for up to 48 hours.
- 4hours
To continue after a reply
When the patient replies, you have 4 hours to continue the case.
- 3hours
First call on a lab result
When a lab result arrives, the clinician who ordered it has the first 3 hours to close that item; after that another eligible clinician may take it.
- 90days
One fill, at most
Each prescription is one fill of up to 90 days with no refills. Controlled substances are never offered.
Signed means final
Each signature signs an exact version of your note; signed records are never edited, and only you can amend or void your own prescriptions.
One posted price. Paid for decisions.
Patients pay one posted price when they submit a request. The platform collects it as a disclosed collection agent for the practices that review it.

The rules behind the share
- Split among the signers
- The professional share is split equally among the clinicians who sign a prescription on the visit, and each portion is payable to that clinician's practice. If nobody signs, it goes to the clinician who made the last clinical no or lab order. Taking or passing on a case earns nothing by itself.
- One $5 reserve
- Asking for information or ordering labs sets aside one $5 reserve per visit from the professional share. It goes to the clinicians who close the lab items, split equally (or, without labs, to those who decide after the patient replies). If ordered labs never come back, the platform keeps it.
- When the fee is kept
- Once you make a decision (a signature, a clinical no or a lab order) the fee is kept, even if items go unanswered later. The patient is refunded in full only when nobody takes the visit, every item ends as a concierge or emergency item, or they do not answer a question within 48 hours, in each case before any decision.
- Second opinions
- A second opinion is a new visit priced on the declined items with the same ladder and the same $10 fee; the clinicians who declined them cannot review it.
- Concierge visits
- Concierge visits for new or closely monitored problems are priced by the reviewing clinician at $50, $75 or $100. Those funds are held for the accepting practice; their split has not been set and nothing is paid out until it is.
- Voiding a prescription
- If you later void a prescription, the patient is refunded the price difference from your practice's share.
- Weekly settlement
- Settlement is weekly (Monday cutoff, Pacific time) by transfer to your practice's Stripe connected account, once a visit's reviewers are final. The platform bears card-processing fees.
This summarises how the software allocates fees. Your participation, the professional fees and the payment terms are set by your practice's participation and agency agreements, and live payments start only after the required legal and payment approvals for each state.
Licensed, insured and independent.
- MD
- DO
- NP
- PA
- Physicians (MD or DO), nurse practitioners and physician assistants licensed in a state the platform serves.
- A nurse practitioner who prescribes under a collaboration agreement is welcome; the agreement goes on file, with no per-prescription cosign.
- A physician assistant whose state requires a physician to cosign each prescription cannot take cases in that state.
- You work through your own independent practice, or join a practice that already participates. Joining never transfers ownership of your practice.
- Current malpractice coverage, and a short scripted practice case in each state before your first patient there.
- In Washington, clinicians who join now start outside the shared queue until the platform's queue manager adds them.
Four steps to your first case.
- 1
